Why Does Sugar-Free Gum Cause Bloating?
It's 11pm, you're bloated again, and you've replayed the entire day in your head. Breakfast was fine. Lunch was fine. You didn't go near onion or garlic. So why does your stomach feel like a balloon? Here's the thing: there's one suspect nobody ever writes down in a food diary, and it's the gum. The three or four pieces you chew on the drive, at your desk, after coffee. Sugar-free gum is one of the most concentrated sources of a FODMAP group most people have never heard of, and the habit hides in plain sight because it never feels like eating.

Polyols are sugar alcohols: sorbitol, mannitol, xylitol, maltitol, isomalt. They taste sweet, they don't feed the bacteria that cause cavities, and they're cheap, which is exactly why nearly every sugar-free gum on the shelf is built on them. The tell is the name. If an ingredient ends in -ol, it's a polyol.
Now the part that matters. Your small intestine has no dedicated transporter for polyols, no doorway built for that molecule. They seep across the gut wall slowly and passively, and a large share of the dose never crosses at all. Whatever stays behind keeps traveling, still sweet, still doing chemistry.
What osmotic actually means when it's happening to you
Water follows sugar. That's the whole mechanism. Undigested polyol sitting in your intestine pulls water out of the surrounding tissue and holds it there to dilute itself. It's the same principle behind the osmotic laxatives on the pharmacy shelf, which is not an accident.
All that fluid has to go somewhere, so it stretches the gut wall. Stretch is what you feel as pressure, gurgling, and a waistband that stopped fitting by 4pm. Further down, colon bacteria ferment part of what's left into gas, which adds volume on top of the water. One small piece of gum, two mechanisms running at once.
And the timing hides it. Polyols take hours to travel far enough to make noise, so the bloat lands long after the gum went in the bin, and you go hunting for a culprit at dinner.
One piece is nothing. A pack is a dose.
A typical stick of sugar-free gum carries roughly one to two grams of polyol. On its own that's a rounding error for most guts. But a pack-a-day habit is ten to fifteen pieces, and polyols have one of the lowest tolerance thresholds in the whole FODMAP family. Plenty of people are into noticeable territory in the single-digit grams.
Then there's stacking. Your gut doesn't assess foods one at a time, it deals with the running total in the pipe. Mushrooms at lunch, a couple of plums, a sugar-free mint after dinner, gum all day. Each one looks harmless alone and they all pour into the same pool.
Gum has a bonus mechanism too. Chewing means swallowing air, over and over, for hours. Some of that air ends up in your stomach and intestine, one more reason a gum habit shows up as bloat with no meal attached.

Once you know to look, polyols are everywhere: mints, sugar-free hard candy and chocolate, cough drops and throat lozenges, no-added-sugar gummies, diabetic sweets, sugar-free coffee syrups. Maltitol and isomalt do most of the heavy lifting in sugar-free chocolate, and a few squares can carry more polyol than a whole day of gum.
Protein and keto bars deserve their own warning. Many are sweetened with maltitol and then bulked out with chicory root fiber or inulin, which is a fructan: a different FODMAP group entirely, and one that ferments hard in the colon. That's two groups in one bar you picked up because it looked like the sensible option.
The front of the pack tells you what a marketer wants you to notice. The ingredient list tells you what your gut is actually going to meet.
The sweeteners that aren't polyols
From a FODMAP standpoint the intense sweeteners are a different animal. Sucralose, aspartame, stevia, monk fruit and saccharin aren't fermentable carbohydrates, and they're used in such tiny amounts that there's nothing meaningful for colon bacteria to work with. Ordinary table sugar is fine on this axis too: sucrose is glucose and fructose in balance, and that balance is what lets it absorb cleanly.
Erythritol is the interesting middle case. It's technically a polyol, but most of it is absorbed in the small intestine and passed out in urine rather than traveling onward, so it behaves quite differently from sorbitol or maltitol. At large doses the surplus still reaches the colon.
None of this means gum is banned for life. It means the dose is finally something you can see: two pieces instead of ten, or a mint sweetened with something that doesn't end in -ol, changes what your gut is handed.
