What's Actually in the Capsule: Supplements, Medicines and FODMAPs
You've rewritten the shopping list, dropped the garlic, learned which stock cubes are fine — and there are still days your food diary can't account for. Before you go through the fridge a fourth time, open the cupboard where the pills live. Not because a supplement or a medicine is doing something to you, but because everything in there is made of ingredients, and some of those ingredients belong to the same five groups you took off your plate. This is a composition guide: what's inside the capsule, why the format matters more than the active, and where the questions stop being about food.
Why the cupboard belongs in the food diary
A diary that records only food has a hole in it. The multivitamin, the sachet stirred into a glass of water, the chewable that lives in a desk drawer — all of it travels the same route as dinner, and all of it is built from the same catalogue of sugars, sweeteners and fibers you've spent weeks sorting through. If a pattern refuses to appear, one possibility is that the thing shaping it was never written down.
Keep the framing straight, though: this is composition, not blame. Nothing on your shelf gets a verdict here. What's in a product is a fact printed on the back of the box; what anyone should do about it is a different question with a different kind of answer, and the last section is about who it goes to. Size isn’t the reason to look, either — a swallowable tablet is usually a fraction of a gram of material, and even an effervescent one is only a few grams. Rhythm is. Supplements are taken daily, often several at once, in a routine so automatic it stops registering as something consumed, and daily-and-invisible is exactly the shape of the thing a diary misses.
The active is usually not the FODMAP part
Take any label apart in two pieces: the active — the thing named on the front, the reason you bought it — and everything else that turns that active into a swallowable, chewable or dissolvable object. Almost always, the front of the box is the boring half of the FODMAP story.
Creatine monohydrate is one small molecule with no fermentable carbohydrate on it. Collagen peptides are fragments of protein. Magnesium, potassium, sodium and zinc arrive as mineral salts. Digestive enzymes are proteins. Probiotic strains are bacteria, counted in cells rather than grams of fiber. None of those are the short-chain carbohydrates the five FODMAP groups describe, so by their own chemistry they sit outside the framework.
Two actives that get argued about constantly are worth stating precisely. Magnesium citrate is magnesium paired with citric acid — a small, highly water-soluble salt; magnesium glycinate is magnesium chelated to the amino acid glycine. That's the whole difference at the chemistry level, and neither citric acid nor glycine is a fermentable carbohydrate, so choosing between them isn't a FODMAP question. Polyethylene glycol, also written macrogol, is a long synthetic polymer rather than a sugar; gut bacteria don't ferment it, so it isn't a FODMAP either. In both cases the composition question lands somewhere else: on the flavored drink powder, the gummy, the ready-mixed sachet.
That's genuinely useful, because it narrows the search. When the same nutrient in two products shows up differently in your notes, the difference is almost never the molecule on the front. It's the sweetener, the fiber blend, the flavor system — the part of the label printed in the smaller font, which is also the part nobody markets.
Format decides: capsules and powders vs chewables, gummies and effervescents
A capsule has one job: hold powder until it's swallowed. The shell is typically gelatin or a cellulose derivative, and inside sits the active plus small amounts of bulking and flow agents — rice flour, cellulose, lactose, magnesium stearate, silica. A plain unflavoured powder is much the same story. Nothing in either format has to taste of anything, which removes the main reason a formulation would carry sweeteners for flavor — though polyols still show up in plain tablets as bulking agents.
Change the format and the ingredient list changes with it. A chewable has to be palatable and hold its shape, a gummy needs a gel base with body and sweetness, an effervescent tablet has to dissolve, fizz and mask a mineral taste in water. Those jobs are frequently done by polyols — maltitol, sorbitol, xylitol, isomalt and mannitol are the usual names, and they're the same molecules the polyol group of the diet is about. The sugar-free versions make this easy to see on the retailer’s own panel: creatine gummies built on a base of maltitol syrup, isomalt, sorbitol or xylitol; effervescent electrolyte tablets listing sorbitol second, which by weight order is not a trace; alpha-galactosidase tablets carrying mannitol among the inactive ingredients. The active is unchanged in each case. The vehicle carrying it isn't.
Plain tablets have their own signature ingredient, and proportion matters here. Lactose is one of the oldest tablet diluents in the business — it presses cleanly and keeps well, so it sits in the inactive ingredients of a great many tablets and capsules. How much is in any given unit varies widely by product, from a few milligrams upward, and it is a name worth recognizing on a panel rather than a dairy portion in disguise. If lactose is a known issue for you, a pharmacist can check the product information and tell you whether a lactose-free version of the same active exists.
So when a product exists in more than one format, the formats aren't interchangeable at the ingredient level. Powder, capsule, gummy and fizzy tablet are four different panels of small print for the same thing on the front, and comparing them is a two-minute job.
The "prebiotic" line, and the trap in plain sight
The most common way FODMAPs enter a supplement routine isn't as a hidden filler. It's as the headline feature. "Prebiotic", "gut health blend", "fiber matrix", "feeds your microbiome" — those phrases cash out, on the panel underneath, as inulin, chicory root fiber, oligofructose, fructo-oligosaccharides (FOS) or galacto-oligosaccharides (GOS).
Those are the fructan and GOS groups under another set of names: the same family of molecules as onion, garlic and wheat on one side, beans, lentils and cashews on the other. Published FODMAP research classes them as oligosaccharides whether they arrive in a bowl or a capsule — a molecule doesn't change category because it was bought in a jar with a leaf on the label. They travel further than you'd expect, too, turning up in greens powders, electrolyte sticks, meal-replacement shakes, protein blends and plenty of probiotic capsules, where pairing bacteria with a fiber to feed them is sold as a feature.
None of that makes the ingredient bad. Inulin is a well-studied, widely eaten fiber with a long history in food. It's simply an oligosaccharide — and if you've spent weeks taking oligosaccharides off your plate, it's worth knowing there's one in the cupboard. Label words that mean "check this"
Ingredient panels are legally required but not written for your benefit, and the same molecule wears several names across products. A small vocabulary does most of the work. Names ending in -ol are often polyols: sorbitol, mannitol, xylitol, maltitol, lactitol, erythritol. One catch on a vitamin panel — tocopherol, cholecalciferol, ergocalciferol, retinol and inositol also end in -ol and are vitamins, not polyols. A few polyols break the pattern too, among them isomalt, hydrogenated starch hydrolysate and polyglycitol. So the ending is a strong signal to look closer, not a complete list and not a verdict. On the fructan side, watch for inulin, chicory root or chicory root fiber, oligofructose, fructo-oligosaccharides and FOS; "root fiber" and "agave fiber" belong to the same story. For GOS, look for galacto-oligosaccharides, GOS spelled out, and fibers derived from legumes.
Two phrases deserve their own reflex. "Prebiotic" never tells you which fiber, so it means go and find out. And "sugar-free", "no added sugar", "low sugar" or "keto" on anything chewable, fizzy or flavored means the sweetness came from somewhere else — polyols are the most common answer. Read those words as a prompt to look further, not as a conclusion.
Two mechanics of the panel itself are worth knowing. On foods and on a supplement’s other-ingredients list, names run in descending order by weight, so something sitting second or third is a substantial part of the product. Medicine excipient lists usually are not ordered that way, so position tells you nothing there rather than a rounding error. And the split between "active" and "inactive ingredients" — sometimes called excipients, or "other ingredients" on a supplement — is a regulatory statement about pharmacological purpose. It says nothing about whether your digestive system notices the ingredient. Inactive is not a synonym for absent.
What belongs with your pharmacist or doctor, not a food app
The useful move is to arrive with better questions. A pharmacist can pull the full product information — the package insert or summary of product characteristics — which lists every excipient, including ones that never make it onto the outer carton. "Does this active come in another format?" is a fair question, and the answer is often yes: the same medicine may exist as a plain tablet, a capsule, a sachet or an unflavoured version. "Is there a lactose-free formulation?" is a standard pharmacy question with documented answers. So is asking which of two products with the same active has the shorter excipient list.
Two lines are worth keeping firm. For anything prescribed, an ingredient list is never grounds to change what you take, skip a dose or substitute a product on your own — the prescription exists for reasons this article knows nothing about. And a dietitian is the right address for the rest: whether your fiber intake is where it should be during elimination, and how anything you take daily fits around a structured reintroduction.
