Why Does Milk Cause Cramps but Cheese Doesn't?

It is a specific kind of unfair. A milky coffee at 9am and you are folded over your desk by eleven, but you can work through half a cheese board at a party and feel completely fine. So which is it: is dairy a problem for you or not? Here's the thing: milk and a wedge of aged cheddar are barely the same food once you look at the one ingredient that actually matters. It isn't dairy doing this to you. It is a single sugar, and cheesemaking takes almost all of it out long before the cheese reaches your plate.

Milk's problem sugar has a name

The sugar in milk is lactose: a double sugar, glucose and galactose locked together. Joined up like that, it is too big to cross your gut wall. It has to be snipped in half first, by an enzyme called lactase that sits along the lining of your small intestine.

When there isn't enough lactase for the amount you drank, the leftover lactose carries on intact. It pulls water into the small intestine as it goes, and once it reaches the colon, bacteria ferment it into hydrogen, carbon dioxide, and methane. Extra water plus a sudden pocket of gas stretches the gut wall, and the muscle responds by squeezing. That squeezing is the cramp, usually somewhere between thirty minutes and three hours after the glass.

Lactase quietly retires as you get older

Babies make plenty of lactase, because milk is the entire job description. For most adults on the planet, production winds down after early childhood. That is the default human setting, not a malfunction. Some populations kept making it at full strength into adulthood, thanks to a genetic variant that spread with dairy farming.

The wind-down is gradual, which is why milk that was fine at fifteen can turn on you at thirty-five with nothing dramatic in between. Dose matters as much as the enzyme. A splash in coffee is a couple of grams of lactose. A bowl of cereal or a large latte is a glass of milk.

There is a second layer if you have IBS. A gut that is already sensitive registers the same water and gas more loudly than a calm one. Same lactose, louder signal.

Why hard cheese is basically a different food

Lactose is water-soluble. It lives in the watery whey, not in the fat and protein of the curd. The first real step of cheesemaking is separating the two and draining the whey off, and most of the lactose leaves the building right there.

Then the starter cultures eat most of what's left, turning it into lactic acid. That is where the tang comes from. Add months of ageing and there is almost nothing left to ferment in your gut. Aged cheddar, parmesan, gruyère, swiss, aged gouda, and manchego carry only trace amounts per serving. Brie and camembert are low too, despite being soft, because cultures ripen them as well. Butter is nearly all fat, so it barely registers.

The wet, fresh cheeses keep the whey, so they are the opposite story: ricotta, cottage cheese, cream cheese, mascarpone, and fresh mozzarella all sit higher. Processed slices and squeezy spreads often have milk solids or whey powder added back in, so check those labels.

Lactose-free milk isn't fake milk

This one surprises people constantly: lactose-free milk is not a plant drink pretending to be dairy. It is real cow's milk, same protein, calcium, and fat. The only difference is that lactase, the very enzyme your gut is short on, gets added at the dairy and splits the lactose into glucose and galactose right there in the carton.

The scissor-work is done before you pour it. That is also why it tastes slightly sweeter, and no, that isn't added sugar. Glucose and galactose simply register as sweeter on your tongue than lactose does. It froths, cooks, and bakes exactly like the milk it came from.

Yogurt sits somewhere in the middle

Live cultures ferment part of the lactose while the yogurt is made, and keep nibbling at it in the fridge, so plain yogurt starts out lower than the milk it came from. Greek yogurt and skyr are strained, meaning whey drained away and took more lactose with it. Same principle as cheese, milder version.

The catch is flavoured yogurt. Fruit purees, honey, high-fructose corn syrup, and chicory root fibre added for gut-health marketing all bring their own fermentable carbs, whatever the lactose is doing. Plain yogurt with your own fruit on top puts you back in control of both.

Tolerance is personal and mostly about dose: hard cheese daily but not a glass of milk, or a splash in coffee but not a latte. Change one thing at a time so you actually learn which is which.
